Please order by completing the form below. Place slip and money in envelope, write player’s name on outside. Cheques made out to 'Alvin Ashfield' will be accepted, however, due to the increased number of NSF cheques, orders will be delayed for two weeks from date cheque is cashed and it will be the responsibility of the ordering person to pick up the photos if later than the team delivery. A portion of the price of the basic combination will be rebated to CAMFA to support the program.

Note:  Player cards are wallet size, printed on heavy stock with the player's photo, name, uniform number, position, team and league name on the front, with additional data printed on the back of the card. Calendars are laminated.
It is suggested that this form be handed out at least a week (or more if possible) prior to the photo session so that parents can be better prepared and make arrangements to have their child present for the photos.
Payment is requested at the time of photography; delays in payment will result in delays in delivery. Arrangements may be made for children missing the photo session to have their individual photo taken at a later date but they will not be in the team photo except by special arrangement and additional cost (currently $15.00). 
A rebate is offered to the league when two or more teams are photographed at the same session.
